Evaluation Process

 

Permission to Evaluate (Includes Gifted only student being evaluated for a Chapter 14 Disability)

 

Parent verbally requests testing

 

1. Complete and send:

a. Permission to Evaluate-Evaluation Request

b. Procedural Safeguard Letter

c. Procedural Safeguard Notice

 

Send all three documents, waiting 5-7 days between attempts, at least three times to attempt a signature.

Document all contacts in Parent Call Log.

Once completed and returned, enter Date of Receipt, and upload the signed document in IEPWriter.

Within 10 days the building staff will consult with the Psychologist regarding the appropriateness of the request. A conversation will be collaboratively conducted with the parent about the appropriateness of this request.

 

Outcomes:

Complete and issue Permission to Evaluate- Consent (go to #2)

Complete and issue the NOREP to rescind (go to #3)

 

2. Upon return of the signed request complete and send to the parent: Consult with Psychologist regarding the assessment tools and procedures that should be included on the Consent form

a. Permission to Evaluate-Consent

b. Procedural Safeguard Letter

c. Procedural Safeguard Notice

 

Send all three documents, waiting 5-7 days between attempts, at least three times to attempt a signature.

Document all contacts in Parent Call Log. Once completed and returned, enter Date of Receipt, and upload the signed document. If an IEP team decision is made to rescind after receiving the parent signature, on the request or consent, complete and send:

a. NOREP (Rescind Evaluation-Request/Consent)

b. Procedural Safeguard Letter

c. Procedural Safeguard Notice

 

Send all three documents, waiting 5-7 days between attempts, at least three times to attempt a signature.

Document all contacts on the Parent Call Log in IEPWriter. Once completed and returned, enter Date of Receipt, and upload the signed document in IEPWriter. Proceed to Evaluation Report (B) if signature on Permission to Evaluate-Consent

B. Evaluation Report (Responsibilities are listed on the separate example document)

1. Oversee the MDE process of completing the ER, scheduling and holding the meeting.

2. Contact the Gifted Liaison for current Gifted only students being evaluated

3. Open and complete the sections assigned and provide access for teachers, including gifted when needed, to complete their sections of the document.

4. Collaborate with the School Psychologist throughout the process, regarding qualifications of need, completion of paperwork and scheduling meetings.

5. Mail the ER document home and an Invitation to Participate before the 60th day.

Team requests testing (Team has collected adequate data)

4. Upload all signature pages.

5. Submit the following signed original documents to the Pupil Services Office

a. Request form/Parent request letter

b. Consent form

c. ER signature page

d. Invitation

e. NOREP

Student does not qualify- Current Gifted Only:

1. Contact the Gifted Liaison regarding the outcome so a Gifted NORA can be issued

2. Complete and obtain signatures on the NOREP- (Did Not Qualify).

3. Submit the following signed original documents to the Special Education ITL/ Program Officer and Gifted Liaison assigned to your building

a. Request form/Parent request letter

b. Consent form

c. ER signature page

d. Invitation

e. NOREP

4. The Case Manager will finalize the RR document in IEP Writer upon review and approval by the psychologist

5. Enter receipt dates when applicable,

6. Upload all signature pages.

Student does qualify:

1. Contact the Special Ed Teacher at the school to complete the IEP within 30 days of mailing the Report.

2. Submit the following signed original documents to the supervisor of special education

a. Request form/Parent request letter

b. Consent form

c. ER signature page

d. Invitation

           

3. Schedule, hold an IEP meeting and finalize the IEP/NOREP/PBSP within 30 days of the date on the Evaluation Report.
